Please complete this form to request a waiver for 2020-2021 chapter dues. If the request is approved, SKCMTA will notify MTNA to remove the applicant’s local chapter dues from their renewal statement. National (MTNA) and State (WSMTA) dues must still be paid. Requests for Chapter Dues Waivers must be received by April 30, 2020 and the waiver recipient must renew their 2020-2021 membership by June 30, 2020.
